Instructions:
Each studio should fill out one Studio Summary Form. Fill out a Regional Entry Form for each entry and be sure to attach a Master Contestant List. You may submit your own contestant list with each entry, but please be sure to provide one list with all performer's names, levels, birthdates and ages (as of January 1, 2013). Please note any male performers on the master so we may arrange for proper dressing space.

These registration forms are in portable document format (pdf) and can be printed out and photocopied. You can also open these forms and type your information directly into each field, then print out each entry and/or save a copy of the forms for your files. You must open these documents using Adobe Acrobat Reader (a newer version) to save a copy of the completed forms on your hard drive.
