Instructions:
Each studio should fill out one Studio Summary Form and include in entry packet. Fill out a National Finals Entry Form for each entry and be sure to attach a Master Contestant List to each entry. You may submit your own contestant list with each entry, but please be sure to provide one list with all performer's names and birthdates. If an entry did not compete at a regional, fill out both a Preliminary Entry Form and a National Finals Entry form and attach a contestant list to each. Please note any male performers on the master so we may arrange for proper dressing space. Dancer of the Year Finalists previously selected at a regional event should be listed on the Dancer of the Year Finalist Entry Form.

These registration forms are in portable document format (pdf) and can be printed out and photocopied. You can also open these forms and type your information directly into each field, then print out each entry and/or save a copy of the forms for your files. You must open these documents using Adobe Acrobat Reader (a newer version) to save a copy of the completed forms on your hard drive.
